For more than 35 years, Brightview of Avon has been providing superb quality care and the comforts of home in a cozy, single-story, 60-bed home. Our Great Room is the perfect place to enjoy elegant, restaurant-style dining, watch television or sit and socialize. French doors open to a beautiful outdoor patio, which includes a secure gated courtyard. We also offer little extras that make a big difference, including a beauty salon, laundry service, cable TV and phone hook-ups. We also offer a “traveling store” cart, which features toiletries and sundry items available at a nominal fee. Brightview of Avon is certified for Medicare and Medicaid.

Brightview is the ideal setting for:
  • Individuals needing long-term, 24-hour nursing care, including those with Alzheimer’s Disease or Dementia
  • Those needing short-term stay for wound care, intravenous therapy or rehabilitation (including knee and hip replacements) due to illness, injury or surgery
  • Hospice for individuals who need end-of-life care
  • Respite for individuals who desire short-term temporary stay
Our Skilled Staff
Our experienced and caring staff takes pride in treating the individual needs of each resident/patient according to their particular functional level. Care of each resident is directed by the attending physician, and our family resident care plan meetings allow loved ones to participate in the plan of care. Our Medical Director is a certified medical director by the American Medical Directors Association. Our team members consist of: physicians, registered nurses, certified nursing assistants, physical, occupational, and speech therapists, a dietary manager, social worker and recreation directors. We also have consulting podiatrists, dentist, optometrist and advance practice registered nurses (APRN) on staff. A licensed registered dietician conducts ongoing assessments of each individual’s nutritional status, and specialized diet plans are provided as appropriate.

Rehabilitation Services
Our licensed therapists provide physical, speech, and occupational therapy for individuals recovering from major illness or surgery, neurological disorders, and orthopedic conditions. Our rehabilitation team works closely with nursing to provide continuity of care and to achieve the highest rehabilitation potential.

Brightview’s physical therapy program focuses on achieving maximum mobility, increasing strength and endurance, and reducing pain through:

  • Mobility Retraining
  • Bracing and Orthotics
  • Therapeutic Exercise
  • Pre and Post Prosthetic Training
  • Cardiac Rehabilitation
  • Balance Activities
  • Ultrasound
  • Home Safety Evaluations Conducted Prior to Discharge

The focus of occupational therapy is to enhance the individual’s level of functioning and productivity while addressing all areas of self-care and daily living tasks through:

  • Splinting
  • Feeding and Dressing Skills
  • Meal Preparation Training
  • Rehabilitative Dining Program
  • Re-Connecting with Contacts in the Community

Our speech-language therapy program focuses on enhancing all areas of the residents communication skills including language, voice, articulation and hearing disorders. We also address dysphagia, which is of critical importance in treating those who have suffered a stroke.

Social Services
Our social worker identifies the psychosocial and spiritual needs of each individual, helps residents and families to adjust to the nursing home environment and facilitates discharge planning. The social services staff works with all departments to develop a care plan to assist residents and patients in achieving their goals, which often include returning to their home following their recovery.

Therapeutic Recreation
Brightview’s recreation program offers a variety of daily activities, seven days per week.
Activities and events include:

  • daily “coffee & news”
  • social gatherings
  • out-trips/shopping
  • live musical entertainment
  • exercise groups
  • arts and crafts
  • word games
  • reminiscence groups
  • non-sectarian worship services Sunday mornings
