Elm Hill Nursing Center is a 90-bed home located in Rocky Hill, Connecticut. Elm Hill
offers skilled nursing services in a warm and home-like environment.

Elm Hill provides an ideal setting for individuals requiring either short-term rehabilitation or long-term care. Following admission, a care plan is created for each resident, ensuring that the highest levels of independence, comfort and recovery are achieved.

Our progressive rehab team assists residents in gaining their maximum levels of function and independence before returning to their homes and communities. Therapists make home visits to assist residents with their transitions, offering suggestions on how to make their environment safe and functional. The recreation department provides a variety of activities including trips to restaurants, museums, movies and community events. We have crafts, gardening, parties and many enrichment programs designed for individual and group participation. Each day our chef prepares nutritious and savory meals, which are served in an elegant dining room.
